DRAGONFLY / Sports Physicals:  Complete your DragonFly profile.

DragonFly Max is an electronic medical records platform that, in the event of a medical emergency, gives DCSD staff, ENTs and medical professionals the needed information to treat and/or transport your student for treatment. All medical documents and sports physical must be uploaded and completed before band camp. All information must be completed and uploaded to Dragon Fly prior to marching band participation.

Getting Started:  Before you begin, read through the information. It takes multiple steps, time, and patience to complete this process if you have not done it before.

  1. Download and print this GHSA PREPARTICIPATION PHYSICAL EVALUATION for All Student-Athletes. It is a “fillable” PDF, and you can either print the blank form, or fill out some of the info (name, address, etc.), then print it for your doctor’s visit.
  2. Make an appointment for an Annual or Sports Physical and take the blank Physical Form (from step 1) with you for them to complete. For a physical to be accepted for the upcoming 2024-2025 season, it must be dated after August 2023.
  3. Once your medical provider has completed the GHSA Physical Form, scan or photograph each page, so you can upload it into the DragonFly website.
  4. Then, visit the dragonflymax.com website and click ‘Log In / Sign Up’ to get started. Our school code is 4VHDT8. Here is the link to the flyer with instructionsUsed DragonFly in the past? Access your account in DragonFly, and simply update/confirm your student’s information, and upload the new GHSA Physical Form. Note: As of March 24, Marching Band was not yet active for submission.
  5. Note:  As you work through each step, the icons to the right of each step will turn green as each is completed. When prompted, upload the new physical, and submit. This section status indicator will turn yellow, and remain yellow until the DHS Athletic Director approves the physical. At this point, you’re done. If everything else is green, you’re done!
